How should I proceed with this bill?

I went to the ER in January. I was checked out and sent home. While at the ER, I had to pay $379 that day. Later on in the month, the doctors and PAs that looked at me billed my insurance. Each sent an itemized list of what the did (which I have copies of) and the price they charged. I got another thing that said what my cost is with insurance and what insurance paid. I paid my part ($1,200) in February. I thought everything was paid for. I got a bill today from “Emergency Care Physician” for $379. There’s no doctor or PAs name on the bill. Theres not even a date of service. There’s no itemized list or diag codes of what I’m being billed for. The only thing it says is “No New Activity – Balance Remaining.” How do I proceed with finding out why this is sent to me? I’ve contacted my insurance provider and they told me they have not received any new claims. Also, I have Regence BlueShield and this bill say my primary insurance is Premera Blue Cross.
