Hospital Coordination of Benefits 2 years later

Hi everyone ! I need a bit of advice on how to proceed regarding an issue that is happening with a hospital bill. On October 2022 I went to urgent care and gave the hospital my primary insurance and I received a letter asking me to update my coordination of benefits in January 2023 because they were charging an incorrect insurance which had ended in Septemeber 2022,before I visited the hospital (I never gave them my secondary insurance because it was inactive, only gave my primary insurance). I double checked with my secondary insurance that benefits had ended before my hospital visit and also double checked with my primary insurance that I was covered during my hospital visist and called the hospital to let them know everything is all good on the insurance side and made sure to update coordination of benefits. Now in April 2024, I am receiving a letter from a third party that I need to update my coordination of benefits yet again. My insurance is telling me they have to reject the claim because it has been over 2 years. Can someone explain what is happening? It seems to me that billing at the hospital is at fault. How can i fight this ?
