Help! Follow up from my situation with short term health plan

Long story short, got laid off, went on marketplace got setup with an agent explained to her my situation but that regardless of future employment I wanted my own PPO plan. She got me this Cigna PPO plan but apparently it’s not a ‘true’ Cigna plan. Anyway, I told her I was going to have a procedure (EGD-camera down throat, had mild gastritis and small ulcer in stomach caused by anxiety, found last year in an EGD) in the coming months. She got me accepted with this plan. $2500 deductible, $250 a month plan. Blah blah.

Now my insurance underwriters are requesting medical records. I have the form in front of me as I have several open claims. Can I just put down the primary physician information from my previous location and exclude the Gastroenterologist who did my EGD? I feel like from the previous thread, if I provide all this info and they see it as a ‘pre existing condition’ they are going to say fuck you and not cover anything…

They’ve covered about 60-70% of all labs and things so far. This EGD was about $9600 all in and so far I paid $2100 up front (basically to meet deductible just about). So if I don’t provide my FULL history is that the way to go? Should I fill it all out anyway and then if they screw me, I go after my agent and get her NPN number and pin her to the wall because she KNEW I was going to have some stuff done yet set me up with this NON-ACA compliant plan…
