Health insurance notified me my doctor is out of network 25 days after the doctor was dropped. Now charging me $200

Hello. My job switched me to a new insurance on January 1 of this year to "save us money". Since then I've had nothing but issues. Here is my latest. I saw my gynecologist in January, February, and March of this year. I was going multiple times due to fertility issues. My latest appointment was April 9. I went in and they asked if my insurance changed. No. It did not. I paid my $50 copay. A couple weeks later I get a bill for $200 claiming out of network. I was confused so I contacted the insurance customer service. As of March 31, 2024 my doctor is out of network. I received a letter DATED April 18 and arrived to my house on April 25 stating my doctor is out of network. This is 25 days after they dropped the doctor. I was told I can appeal the decision. The insurance denied the appeal and is claiming that I need to call before EVERY appointment to confirm in network and that it is not their fault I was not notified that my doctor was no longer in network. I was notified 25 days post drop. No email was sent. It was a letter. They have my email on file because I get monthly ones stating the claims I had each month. But nothing regarding the fact my doctor who I've seen 3 times prior to this drop was out of network or going to be out of network. Are they allowed to do this? How do I make them change their decision? They already hit me with a $150 bill for an infertility ultrasound because it my state it just needs to be covered but not 100%. They covered $100. That's it. My $500 a month insurance doesn't do crap. Please help me with how to appeal this even more. Dates clearly don't matter to them.
