I'm seeking advice on the best plan for my situation.

My medical situation:

In 2017 I was diagnosed with a Sarcoma, which is a rare type of cancer. I was treated in my home state and the treatment (radiation, multiple surgeries was covered completely by insurance (Anthem HMO via Healthcare.gov)

2 years later, I had a recurrence which was treated again and fully covered by the same insurance.

This year, I had a second recurrence and the doctors in my home state HMO network (a specialty Sarcoma center) recommended that I get treated in a out-of-state and out-of-network hospital (Dana Farber in Boston) that could give me access to more advance treatment (proton radiation to reduce the dosage of an area that has already had radiation previously).

After receiving out-of-network approval from Anthem for these treatments, going through the treatments and in recovery the hospital let me know that Anthem refused to pay, leaving me with the bill ($200,000+).

What I'm seeking advice for:

In the future I know that I will continue to have a very high medical spend each year (my follow-up imaging will easily get me to most out-of-pocket maxes). Additionally, I want to be sure that if there is another recurrence, I can get treated at Dana Farber in Boston but not get caught with the bill again.

I am self-employed and have been on HealthCare.gov insurance since about 2016.

What would be your recommendation for my situation?
