Health Insurance Denying Claim Due To Invoice Formatting, Clinic Won’t Change Format

Hi, I have multiple claims with my insurance company I'm waiting for them to approve. The clinic provided me with invoices and payment receipts which I included with my claim.

The insurance company keeps denying or requesting more information due to the layout of the invoice:

The first time the quantities for each procedure (hourly electrolysis prep for surgery) weren't listed clearly. So the clinic updated that with quantities (number of hours).

Then insurance refused to pay if there was a credit on the invoice for unused time from a previous session, so I just gave up on that part and resubmitted without those charges.

Now they say that rather than the quantities they need each hour of electrolysis on it's own line. The clinic has said they cannot/will not provide any additional invoice formats.

All the information is on these invoices pretty, if not perfectly, clear. As someone who doesn't interact much with invoicing they are very legible, and this same insurance has already accepted other claims laid out in the same way.

What are my options here? These bills are very large and I know for a fact I have coverage, I'm being denied on
