Government agencies to report doctors/insurance to

So my girlfriend had a procedure, the doctor who referred her to the place to get the procedure told her it would be tier 1 and covered by insurance. She showed up for the procedure, asked at the front desk and they told her it would be tier 1 and covered by insurance. Talked with the anesthesiologist, he said he was tier 1 and covered by insurance. Gets the procedure, the hospital sends the bill to insurance, they say it’s tier 2 so the hospital sends the girlfriend a bill for 3k.

So I’m advising her not to pay it, I sure as hell wouldn’t, I’d let it go to collections, dispute it, then report every doctor, nurse, the hospital, the insurance company, anyone I can think of, to every government agency that oversees them, for every complaint about the procedure and the associated billing that I have. I’ve done it in other situations but not in regards to medical bills. Where should I start?

I’m also open to other ways of resolving this but talking to the doctors office, hospital and insurance company has gotten nowhere. The hospital just said we can’t prove they told us it was tier 1, and offered to knock 25% off the bill.
