Getting a second bill after paying upfront.

I went in for a simple upper endoscopy for preventative/exploration care after some distressing symptoms. Day of the procedure I walk into the facility where it is to be performed and ask what my copay is: They say $580. I say fine, I pay, I get the procedure and all is well.

Weeks later I receive another bill saying I owe another $560. My insurance has no idea what this claim is. I call the place and ask and they tell me it's a separate charge for use of the facility, separate from the cost of the procedure. I ask why I wasn't informed of this cost upfront day of the procedure and they have no answer and just say they expect me to pay it. My insurance says their part in this is over and have nothing to add except that I should probably pay the facility and they aren't covering anymore of this procedure.

What the hell is this? Legal?

This has been happening a lot lately where I feel like I keep getting bait and switched by healthcare. I ask what the costs are, I pay upfront, and then later I get more bills anyway. I feel like I'm being fucking scammed.

I'm relatively new to dealing with all of this. For the past decade I had union-provided insurance and everything was just paid for, no questions asked. I guess I was spoiled because this is bullshit.
