Fraudulent–When, where or who adds/applies medical insurance to a patients account? Is it typically only done from the Dr office visited?

Received a notification about a Dr visit bill stating that BlueCrossBS-OK paid $X on 6/27, insurance discount of $X and what I had to pay. Problem is I currently do not have medical insurance. I called the medical facility and under my name, DOB, phone# was this BlueCross of Oklahoma listed as my primary insurance. They even provided me the group & member id numbers so I called BlueCross. That policy isn't not in my name, it's a Medicare plan and it was terminated in 2017 according to them but supposedly paid part of my bill.
BlueCross called the medical facility 3way with me and they were just stumped. They couldn't tell me when this insurance was added to my account nor could they remove it. Said I'd have to call the Dr office to do so. (They're closed on Fridays)

Thing that's got me going is there's a nurse at this clinic I was talking to for a minute but thought better of it so I stopped talking to her. 2 weeks later I got a letter saying I was released from their care and won't be seen 30 days after receiving the letter. And now I have someone else's insurance on my account making fraudulent payments.
I would love to figure out when, where, how and who added that insurance but I'm not sure I'll ever find out.

And yes in a small one horse town in Kansas things can be just that dumb without more to the story.
