Fighting for preventative care when it’s charged as diagnostic?

My doctor and nurses have taken great care of me during an unexpected twist in my IUD replacement. We are both struggling with a siloed, byzantine administrative system, and I’m not even sure I’m fighting for the right things. I need your help determining how to proceed. I’m in New Jersey under a UHC PPO plan. My OBGYN’s clinic and the hospital are in-network.

TL;DR:
My IUD removal, insertion, and anesthesia are all charged under ICD code T83.32. The IUD was displaced and overdue for removal, but I wasn’t in any pain.

I want to know if I can rightfully have the hospital bill my IUD insertion and anesthesia under Z30.430, keeping CPT code 85300, and removing 58562. (These are what I see on my CMS-1450 that was accidentally sent in lieu of an itemized bill.)

The hospital and OBGYN clinic have done the following, and I want to know if I should take legal action:

Failing to write the proper ICD and CPT codes on the surgery scheduling form

Changing ICD codes without the doctor’s approval

Neglecting to address uterine cysts

Scheduling appointments over the phone and incorrectly adding the appointment times to MyChart three times

Failing to respond to the patient after requesting them to call their doctor (I had a fever and shortness of breath for about a week after the surgery, and I had to call my doctor four days before someone said I could only talk with them if I pay.)

Failing to write the correct dates down for the surgery on the bill (I was billed as if the appointment lasted three days, not 8 hours)

The long, detailed story:
I met my doctor in May for my first visit with her. She told me I was a year overdue for an IUD replacement and that she couldn’t see the strings. I would need a sonogram in advance, and I should have the replacement under anesthesia.

No department seemed to understand what my doctor and I discussed, but we eventually scheduled the surgery for August.

Billing calls in June/July to give me the CPT codes for the procedure. I’m told it will be billed as preventative.

I have a sonogram and nobody discusses the results with me. I later see the results on MyChart and see they found an intramural/subserosal fibroid, a simple cyst, and a hemorrhagic cyst. The largest dimension of any cyst is 2.9cm, so I assume it’s normal. Someone would tell me if there’s a problem, right?

Billing calls less than a week before the surgery. Their new estimate is $20,000. They’ve added CPT code 58558, hysteroscopy with biopsy or polypectomy. The ICD code for the entire procedure is now T83.32, displacement of intrauterine contraceptive device, initial encounter. I call the clinic and my doctor says there’s been a mistake and will remove the 58558 code from Epic and add the ICD code Z30.430, encounter for insertion of intrauterine contraceptive device.

Lab services calls me three days before my surgery and says they need me to come in at 8am the next morning. It’s a workday for me. I’m shocked but have no other choice.

On the day of the surgery, patient check-in tells me I’ll owe $20,000 and CPT 58558 is still on the list of procedures. Z30.430 is missing. I tell my doctor while I’m on a gurney, who writes what we’re doing on her clipboard. No biopsy or polypectomy. However, she writes T83.32 as the only ICD code and CPT codes 58562 and 58558. I didn’t realize what she’d written until today.

In my follow-up appointment with the doctor, she was shocked to see someone had changed her ICD codes. She showed me where it had changed and said she’d never seen this happen before. Her manager said over the phone that all IUD removals are charged under T83.32, which I think was said because she is stressed out and under pressure. There’s no way that’s true.

UHC said they’ll request the hospital to reprocess the claim because it spans three days and may not have the correct codes, but I don’t feel like anything will change without the hospital speaking with me.

I feel lost and confused. Nobody seems to be taking responsibility for what’s happened.

I also feel bad for the people working at this office. It seems like chaos and confusion all day. No wonder they defer my questions to someone else.
