Family member bought insurance from a broker; want to verify that this is legitimate

A family member bought insurance through a broker that he got from one of those “Get Insurance Now!” websites.

We checked the broker through the NAIC website.

And called the number on the profile and he picked it up, so the broker seems legitimate.

He signed up for a 1-year plan for “First Health network Limited Benefit Plan” Advantage 6. The card they sent him by email only lists First Health network LBP and Multiplan LBP Complementary. There isn’t any regular insurance company on the card.

It says to submit all claims to HMA, LLC PO Box 22009 Tempe, AZ. That seems to be a Navajo reservation?

It also came with an Rx membership to America’s Pharmacy Source and Advantage Starter Dentachoice Plus for dental. The dental was advertised as not dental insurance and not a big deal.

It’s a PPO plan. $25 copay for primary, $50 for specialist, and $8550 out-of-pocket maximum for in-provider.

We didn’t receive a schedule of benefits yet or paperwork like that.

Does this seem like a real insurance plan or a scam?
