ER bill received since meeting OOP help!

Hello, so I'll make this quick. Went to ER due to Crohn's in January. Afterwards got process rolling to get Humira prescription. Finally got approval though insurance, medicine is 100% price until I meet deductible so I had to shell out $5,900 for the medicine (it is a tier 4 specialty medication)My OOP is 9,000. So I paid that, then many other health things have happened since then, got a colonoscopy, urgent care visits and continued paying for Humira. I am now at 8,000 of my 9,000 OOP. Just received bill from ER that I owe 3k for the visit. So how does this work? I KNOW for a fact I've paid 8,000 and that my Humira medicine COUNTS towards deductible (literally says it in plan.) insurance is adamant the 3000 I owe i still owe because that was part of my what met my deductible but I literally have credit card proof I've paid over 8k already. What am I supposed to do? Someone needs to refund me if I technically still owe 3000 but who since the ER visit was first but I already paid my medicine? Just so confused and no one there is any help at all.
