Employer Isn’t allowing me to enroll. Is this legal?

I began work for my company in April this year, and once I hit the end of my 90-day probationary period I was given the information to enroll in the health insurance benefits provided by my company. Upon making my account however I found two things; First was that nearly all my personal information was incorrect. My name, DoB, Social, address, and phone number were incorrect. Second was that I had already been enrolled in life insurance benefits, and that I had no option to enroll in any other program. A week ago, I reached out to our HR department and have since received no correspondence. I called the insurance broker today, who informed me that forms had been filled already and submitted. I told her that I never did this to which she replied, “Well someone did.”

Is this legal? In two weeks I lose all eligibility to enroll my wife and I, and this is health insurance we really need. What other options are there I can look at?
