Employer ghosted me, how do I apply for COBRA without them?

I was laid off Monday last week. My employer is having massive financial issues, so they need to minimize overhead as much as possible. They did the usual "let us know any way we can help!", and then removed my access to my work email, stopped responding to emails from my personal address and won't pick up the phone (also didn't get my last paycheck but that's another story).

With the month ending i'm worried about my health insurance lapsing, but I have no faith my employer is going to send me the documentation they are supposed to in order to sign up for COBRA. Everyting i'm finding on google mentions the COBRA process starting there, anyone have ideas on other options? or if my insurance will continue for the "up to 45 days" my previous employer has to send me the documentation.

I figure calling my insurance company might answer these questions, but it also might "tell on myself" resulting in them canceling my insurance before they otherwise would have been notified to.
