Does anyone know why I am being charged more going thru insurance vs self-pay?

Got my husband who is newly diagnosed with Autism/ADHD to see a psychiatrist for medication treatment. The psychologytoday.com profile said it was $150 for a visit. my husband was laid off due to his conditions so we are living paycheck to paycheck on just my income. Its already stressful without insurance worries. I added him to my insurance so now we both have a BCBS Ppo high deductible plan that we obviously havent met his deductible yet because he just got on it ($5000 ind.). I told the doctor about our situation and he said it shouldn't cost more than $250 for the appointment since its the initial appt, more than $150 but i guess i can understand because its an hour long appt vs 30min. But I got the bill and it was $350. I emailed his billing lady and she said a lot without explaining anything. I look up the claim in my insurance and it says the claim is for $700 and says there is 3 parts the "Medical Visit" and two "Therapy". Why am I paying more than what he told me? I have been emailing back and forth and theres no getting a straight answer. Its bad enough they're using Headway which doesnt let me split my payments so I can't use my HSA card. Am I being scammed or do all doctors charge insurance 3x times the amount they say? She said I could switch to self-pay but then it wouldnt contribute to my deductible so whats the point?
