Does an ophthalmologist need consent to do the procedures beyond an exam?

I went for a light sensitivity visit on advice of the allergist. I last went to ophthalmologist in 2019 so I guess I was not recurring patient and I dont recall my fees in 2019. I pay a 45 copay anthem blue cross for a visit and 20 percent of all procedures. So the technician checks me out doesnt say anything about what they are doing and then later that was billed as an ophthalmoscopy with retinal and sceiral [code 92201]. I go into a room with a camera and ask the technician if this is included as part of the exam and he asks and says yes. NOone explains to me what the camera is for or why I need to do any of this and I still have not spoken to doctor about my issues. Later this is billed as a fundus photography code 92250. they also charged me comprehensive eye exam new although I was there in 2019. 92004 code. Upon seeing the codes when I was leaving I became concerned why I was charged additional procedures without my consent or anyone even explaining what was being done. Is this doctor’s office acting inappropriate by not counseling patients on the procedures and finding out if patients consent- or is it okay for them to charge me for additional procedures and not inform me; is it my fault for not asking ahead of time and making sure that i was only being charged for an office fee by the admins. Apparently, they dismissed the technician’s info to me saying he is a college student and gave me wrong info.
