(TEXAS) Is a doctors office able to get ahold of your insurance information even though you never provided it?

Context- I had a virtual consultation set up through my company for a possible surgery I was considering.

During this whole scheduling process I was told not to provide my insurance information because our company goes through a third party to fully pay the bill if it is scheduled and set up through said third party.

I was given a voucher to give to the doctors office and on it it specifically states “Do not collect payment/insurance information from patient”.

Almost a year later from the initial consultation I received a bill from said doctors office showing my insurance was in fact billed and I had a remaining balance to pay… how is this? Did the doctors office find my insurance information through the personal information i provided them?? I never provided my insurance information and my insurance was still billed for it.
