Doctor called in wrong dose for a medication I’ve been on for years.

I recently started seeing a new doctor and when I went to my appointment, I had a list of all the medications that I needed a refill. Not only did he not call the medication in till the following day BUT he forgot two of the medications that I needed. After numerous rounds of phone tag, I finally got ahold of someone at his office and they said he was going to send them in.

I picked up my medication today and when I got home, I realized that he not only forgot one…again BUT he gave me the child's dosage of an emergency medications. I called the pharmacy and they confirmed he called in the wrong dose and was told I'd have to call the doctor again and have it called in.

I'm planning to call my insurance(BCBS of IL) on Monday to explain the situation. Who is at fault here? The doctor? The pharmacy? This is something I have been on for years with the proper dosages in my records. Insurance will only cover a certain number of this medicine a year with a $15 copay. Either way, I am not paying hundreds of dollars for someone's mistake.
