Disability claim denied. Need help

I got injured playing sports on April 28th and from then on I haven’t been to work due to the pain. I didn’t see my family doctor or do any test untill June 12. This was mainly due to 1)financial reasons: not able to pay for doctor’s documents. 2)My family doctor is terrible so I was looking to find another doctor. I applied for short term disability with my jobs benefits insurance when I finally went to see a new doctor (June 12) as well as did an X-ray and booked an ultrasound for June 28. Today I got a call from my insurance saying that my claim for disability has been denied because when I got injured, I didn’t see a doctor right away, never did any tests till june 12 and didn’t do physio. I told the agent the reason I didn’t go see the doctor right away or do any tests or physio was because I didn’t have money to cover the costs. But she still didn’t care and said if I really was in pain I would’ve seen the doctor. The agent said that I can appeal this decision but I need to give a good reason why it should be appealed. Can you please provide me with some good reasons so I can get this decision reversed? Also what can I do about all this? I was really relying on insurance for payment since I haven’t worked since April 28 and I’m broke and don’t have any money for my bills
