Didn’t get prior authorization for a CT scan (my fault)… am I screwed?

I got a CT scan in 2021 – at the time my insurer didn't require a prior authorization for any CT scan.

My doctor sent me to get another one yesterday. When the imaging place called, I told them not to worry about the prior authorization since it was no problem last time. I had a fairly uncomfortable lump in my stomach (likely a hernia) and just wanted the scan done and results ASAP, not wait around 5-10 days for permission. The imaging place said OK – but had me sign a financial waiver.

Well I double checked today and it look like in 2023, CT scans were added to the list of services requiring a prior authorization. Am I totally screwed now?

Last time the scan was $600 which insurance "covered" but didn't really matter since we didn't hit the $4,000 deductible, so I basically paid $600 out of pocket. Chances are we won't hit the deductible this year (hopefully), but in the event we do, then I just kinda screwed myself out of $500+.

If insurance doesn't cover it, can I ask for a cash discount? Is it possible to get retroactive authorization?
