Did I unknowingly commit fraud? How do I fix it?

So, I have an HMO plan and I ended up getting really sick in January and leaving my home state to stay with my support system while I got better. I had an apartment at my address in my state until end of March. I ended up leaving because with all the health issues I didn’t feel safe being there alone. Since then I’ve been homeless and traveling between states, including my original state. My insurance offers virtual care so I was using that, as well as ER visits (also covered while traveling), and I established a temporary health record number in one of the states I visited. I’ve also had specialist appointments in my home state.

Was I meant to inform my insurer about this change in living situation? During at least two of my telehealth appointments in March I told my doctor I was away and with family. I’ve recently been asked to provide an alternate mailing address and didn’t understand why so I didn’t give one. But now that I’ve read threads here on Reddit, it’s dawned on me that I may have done something wrong.

What do I do now? I’ll likely remain homeless and couch surfing for the rest of the year. Do I call and explain? Should I cancel my plan? Will I be in legal trouble? I’m really freaking out 🙁
