Deductible reset after dependent passes away?

Hello, I could really use some help please. My spouse passed away earlier this year, and he was covered under my employer sponsored plan in FL. I notified my HR and Benefits administrator. I was informed by an insurance rep that because he would be termed from the plan, my deductible and OOPM would reset to zero and updated to reflect an individual vs family. Flash forward a couple of months, I had some unexpected health issues arise. Another insurance agent said that his medical expenses exceeded our deductible and OOPM and because the policy was the same, my expenses should be covered. Tried to share this with my provider/treating hospital but they can find no record of the deductible or OOPM being met. How do I find out specifics of my plan and which scenario is true? I’ve read that sometimes acccumulations can be manually applied when a dependent is termed-just need some guidance on who to speak to in order to get some clarity.
