Covered by insurance but insurance not on file for doctor’s visit

So I am being billed as if I do not have health insurance because the doctor's office is saying that since a copy of my health insurance records were not on file that I am now liable for the bill in full even though I have had coverage this entire time. It happened months ago, I was not notified, and now they are saying it is past their 90 days date. This is their exact message:

Message Hello, Per billing team… New insurance was provided 07/30/2024. Claim for DOS 04/02/2024 & 04/30/2024 are outside timely filing rebill limit (90 Days). Balance will remain Patient Responsibility. Thanks! Thank you —-Original Message————————————- I have a statement billing me for an office visit on 4/2/2024 and also 4/30/2024, was wondering why there was duplicate visits for 255 each as well as why insurance was not applied?

What can I do about this? Can my insurance still cover this or is it my responsibility since it's been over 90 days.
