Coverage for medication off label

Last year I was on mounjaro and the biggest thing I noticed was the relief of my knee pain. Thought it was obviously due to the weight loss. Well a month after stopping due to no insurance coverage the pain came back with a vengeance. I maintained the weight-loss for several months but the knee pain has really impacted my life.

Went to the Dr and have started a new medication for my psoriasis in hopes that it's psoriasis in my joints (it is not Psoriatic Arthritis, already ruled that out) this medication hasn't done a thing for my knee pain.

While I know mounjaro/wegovy is hard to find right now, how hard would it be to get insurance to cover the medication off label when it becomes more available?

Has anyone ever heard of writing a letter to insurance to explain that the medication I want and know works is actually cheaper than the medication they approved and I'm currently on.
