Collective Health/BCBS check reissue taking 4-6 months and counting?

tl;dr: will I ever get a check reissued due to insurance’s error?

This insurance is through Collective Health (third-party administrator) and uses the Anthem BCBS network (Empire in NY state).

I see an out-of-network provider on a weekly basis. Every month, I file a claim and a reimbursement check is issued to me.

In July 2022, the check was issued to my provider in error. She could not accept it for tax reasons and requested that it be voided and re-issued to me.

So I called Collective Health in August 2022 and reported the issue, and was told 4-6 weeks for processing. When that never happened, I called in October and went through the same thing. My provider also called Collective Health. She did confirm the original check was already voided. They also told her 4-6 weeks.

After 6 weeks, still no check — and when I asked, they were still “working on it.” But on 12/15/22 I received a message that the check was going to be reissued in the next 2 weeks and mailed in 5-7 biz days.

Again, no check showed up. This week, my provider called and confirmed the check has NOT been re-issued yet. Supposedly they were/are waiting for something “on the medical network’s end.”

So we agree it’s my money. My question is whether it’s unusual for a check to take this long. And is there anything more I can do? In past experiences, there were fewer middlemen to get something like this done. I have tried calling various BCBS numbers but the reps are unable to pull up my details — likely because of whatever way the third-party admin stuff is set up.
