Claim pended over questionanaire

So, I recently sustained an injury which required surgery. After the surgery was completed, I noticed on my insurance portal that the claim for the surgery was marked "pended" awaiting receipt of a questionnaire.

The first question is "Was the above-referenced claim the result of an accident or injury?"

If "Yes," there's a box for date of accident or injury and a box for location of accident or injury, and a third box for a description of how the injury occurred.

Then there's a second question on whether the accident or injury is covered by other insurance (worker's comp/property/homeowner's/automobile/other).

To me, this is obviously an attempt by the insurance to deny the claim and pin it on another party. The problem for me is, I was very drunk when I sustained the injury, and I'm not actually sure how it happened. It was in a friend of a friend's back yard, but I'm not certain how it occurred. I don't really know how to fill it out, and I'm also worried that if I obtain an address where the injury occurred, they'll try to claim that the property owner is liable.

If anyone has any experience with this sort of thing, any input would be much appreciated. This is in Indiana, btw.
