Claim has been pending for a year. What do I do?

I had a fertility test run in July 2023, and a claim was submitted to my insurance. This was initially denied, as my insurance company claimed that "treatment for fertility is not covered.". But it wasn't treatment, it was testing (which specifically is covered). I contacted my hospital's billing department and they revised the claim.

One of the line items was approved in a couple days, but the other ones have been in a perpetual state of "awaiting response". My insurance says they have not received a revise claim for these. I followed up with the hospital last October who said "The claim is still pending review and payment by insurance. Please allow time for this to process."

Nothing has happened since, and I don't even have that insurance anymore (after changing jobs). What do I do? My fear is that suddenly this is going to hit my account and I'll owe hundreds with no recourse.
