Claim denied and Appeal Rejected?

Hi, I had a heart ablation surgery 3 months ago and insurance is refusing to pay the 6 figure cost of the surgery . Never had this happen before and am confused as to what to do . They deemed it not medically necessary even though a heart monitor showed my heart was out of rythym 20 percent of the time and i have a history of genetic heart problems.. So the claim has been denied and than the appeal was also denied . The last step is a independent external appeal that I have to sign off on. I call the hospital and they say this is normal and that don't worry until u get a bill and ignore the insurance letters . Than I call the insurance and they say don't listen to hospital . they say that at this point in order to go foward i must sign the external apeal or pay. I just don't know what to do and I don't understand why they are rejecting a absolutely necessary surgery l. I mean if it wasn't necessary than why did the dr say my insurance would cover it. And why does the hospital seem to be acting like its no biggie even though the appeal was rejected. I Pay too dam much in health insurance for them to be rejecting this. Any advice??
