Can y’all help me choose a plan?

I just started a new job and will be making 48k a year and I am trying to choose health insurance. I am trying to choose between a $500 deductible and a $1000 deductible plan.

BCBS Louisiana. The monthly premium for the $500 deductible is $304.58, while the premium for the $1000 deductible is $231.22. (I’m in Louisiana — insurance is mad expensive).

Out of pocket max for $500 deductible is $3750 and for the $1000 deductible is $5250.

Copays and coinsurance are the same: $40 copay for primary care, $55 for specialist, mental health care $40, urgent care $55, and labs are fully covered. Inpatient, outpatient, and misc are deductible then coinsurance which is 20%.

I am 26 and relatively healthy but I do go to the doctor fairly frequently. I’m on 5 medications, see a psychiatrist monthly, therapist weekly, gender affirming care 1x yearly, I’m on PrEP so I need blood work done every 3 months, and I am in physical therapy weekly.

Can someone help me navigate this? I don’t have good relationships with any adults in my life to ask these kind of questions. Which plan would ultimately wind up costing me less money? Also, would it make sense to do an FSA with the amount for the deductible in it?Thanks in advance.
