Can my spouse be double insured? And other questions?

I've (M35) just started a new job with a better benefits package from Aetna. My spouse (F35) has a bare bones insurance from Kaiser that her employer fully covers. When we had our child, we got charged two Family Out-of-Pocket Maximums because it wasn't possible to add a child to her plan. (That was our understanding, but it's a 6 person non-profit and the HR stuff is done very badly. The 35K bill all got forgiven by a Kaiser MFA award during the brief moment they were being generous with those.)

It will cost an extra roughly $160/mo to add her to my plan, but we're starting to try for another pregnancy and I think that should avoid the double maximum issue again? Alternatively, how does it work if the mother is on Kaiser and the newborn on Aetna?

The big wrinkle here is that we don't want to lose her therapist, who is out of network for Aetna. So we were wondering if we can just keep the free-to-us Kaiser plan and add her to my Aetna plan?

Or is there another better way to navigate all this BS? Thanks!
