Bluecross Gold PPO in Cal , Moving NYC when should I change plus…

We have a BlueCross PPO Gold plan in California. We are retired and pay for this out of pocket via CoveredCA. We pay ~ $3.3k for a family of 4. I think it is completely obnoxious personally but it be what it be.

I tend to choose PPO's simply because I like the freedom a PPO offers.

We, the wife and I are moving to NYC sometime in October.

We have child that will be starting at NYU in august and though they offer Student insurance when I did the comparison it was cheaper to keep him on our current plan.

Our other son is staying in California and will just stay on our plan for now.

I also use a service called Forward and pay $149 a month. I actually really like this service.

Questions:

1 – Given I use Forward (they do not do speciality , they will recommend a specialist if needed), is there a reasonable insurance combination to use with this that makes sense?

2 – NY has a Health Market Place. I think on this forum I had read that companies are still penalize you for pre-existing, I had kidney surgery last year, I have High Blood Pressure, wife has elevated cholesterol. Will this prevent us from getting Health Insurance.

3 – since I have insurance in California and we are not moving until October should I wait until end of Year before doing the switch?

4 – Should we get an HMO instead to cut costs? We would get rid of Forward at that point as it seems pointless at that point…

If there are any suggestions are welcome, things we should be thinking about etc…
