Dear Member,

We've approved coverage for services from a non-participating facility at an out-of-network benefit level. You'd pay less for these services if you get your care at a participating (in-network) facility.

Out-of-Network Facility Payment Estimate: We can't estimate how much your plan would pay the facility at this time. We were unable to get an estimated charge from the facility. If you can find out how much the facility expects to charge for your planned services, then you can call Member Services to get an estimate of how much your plan may pay.

The tool below may help you estimate your costs if you plan to have other out-of-network services.
