I recently got a full-time job and they're offering me 4 different insurance plans and I have no idea what I'm doing when it comes to picking one. I'm a trans man and am expecting to be using my insurance A LOT (specifically for my testosterone prescription), as well as mental health services for my other medication. My previous insurance was through my university, but it ends at the end of this month since I've graduated. I also at some point want to get top surgery, so I want an insurance plan that's best for that as well. I've also included the out-of-pocket limit and prescription costs.

The 4-plans they're offering me are:

Kaiser Platinum 90 HMO

OOP limit: $3,000

Prescription drug coverage: Tier 4 drug: 10% coinsurance up to $250/prescription

UnitedHealthcare Signature Gold HMO

OOP limit: $7,500

Prescription drug coverage: Tier 4 drug: 25% coinsurance up to $250 copay max per prescription

UnitedHealthcare Signature Platinum HMO

OOP limit: $3,000

Prescription drug coverage: Tier 4 drug: 25% coinsurance up to $250 copay max per prescription

UnitedHealthcare Select Plus Gold PPO

OOP limit: $8,950
Prescription drug coverage: Tier 4 drug: 25% coinsurance up to $250 copay max per prescription

The first 2 plans are at no cost to me, while the last 2 have a bit of a fee, but I don't mind paying since I will be using it frequently and believe it will be worth it. Please please help. I've heard bad things about Kaiser and heard PPO is best but I have no idea.
