Any Help On Understanding these 3 Employer Options?

Hi, so I’m trying to figure out which plan makes the most sense for me. I take expensive medication for mental healthcare and for mental healthcare alone I know it’s going to be over $10k for the year easily.

So I have 3 options

$2012 Premiums/$3000 deductible/$7150 max out of pocket

$1473 Premiums/$2000 deductible/$5500 max out of pocket

$2705 Premiums/$500 deductible/$4000 max out of pocket

There are some % of copays that are different but my question is on the mental health coverage along with these options.

Mental Health options

Medication $30/Prescription Deductible does not apply

Outpatient $20 copay per visit/ Deductible does not apply

Out of network 40% coinsurance

So I know I’m going to hit the max out of pocket on all these and the $2705+$4000 is slightly less than the $1473+$5500.

Is there anything I should know about what would change the max out of pocket? Like if it’s out of network or does the out pocket cost still apply if the deductible doesn’t apply?
