After 2 years, health insurance claim they overpaid provider wants their money

I recently got a bill from my regular provider for service that they provided two years ago. The bill for that service was marked as paid as much as a year and a half ago but now I owe nearly $100. Apparently Aetna has decided that they overpaid the claim, so they clawed back what they called an overpayment and the provider had no choice but to bill me.

Compared with how bad this could be $100 doesn't seem like a big deal except… How do I know they didn't overpay every claim? I've had Aetna as a health care insurance provider for more than 6 years. Am I to live in fear that they will review some claim that they paid years ago and decide they overpaid by some ridiculous amount for which I am now liable?

Also since the date of service on this thing was two years ago, the original service provider has claimed every right to send this to collections. WTAF?

1) Is this legal? 2) What should I do? I don't want my insurance company taking advantage of me in this way … Not now and not in the future.
