3 Bills sent to me but not my insurance. Envision health scam?

I had an ER visit not too long ago and I’m confused why my insurance doesn’t know anything about it.

I got 2 bills from the mail and one from the doctor through envision health/physician services. The 1st bill was from the radiologist asking for $442 for a CT scan. The 2nd was from the hospital for $1442 which had CT scan, EKG, ER Room, Laboratory. And the 3rd from Envision Health/Physcian Services for $1860 for Emergency Evaluation (lvl 5), 12 lead EKG, non-invasive pulse oximetry, rythm strip interpretation.

When I was called to provide my health insurance and personal information, I asked the lady at the front desk if there was any charges that I would be needing to pay for the visit and she told me she didn’t see anything regarding outgoing charges.

After speaking to my insurance they told me there was no claims in the date I had went to the ER.

My concern is also the envision health bill which made no sense to me, why am i being charged $1860 for repeated services from the hospital bill from a doctor i never chose and was told i’d be charged separately from as well only seeing him for 5 minutes for a small evaluation.

I also looked at the code from the envision health bill specifically code 99285, which states:

“Comprehensive History Includes: • Reason for admission • Problem pertinent review of systems • Extended history of present illness (HPI)

Includes 4 or more elements of the HPI or the status of at least three chronic or inactive conditions • Review of systems directly related to the problem(s) identified in the HPI • Medically necessary review of ALL body systems’ history • Medically necessary complete past, family, and social history”

When i spoke with the doctor i told him my family had no history of the similar medical visit im having and i also have had no history of it ever happening to me before which already is way beyond the the description of the code.

What should i do and who should i speak with?
