24F (GA) with multiple conditions looking for insurance advice

Hello and thanks in advance for any help.

I am a 24-year-old female located in Georgia who needs a new insurance plan. Up until my graduation this year, I have been on a plan through my university. More recently, I've had a plan through open enrollment (United Healthcare Gold Advantage+) for the past few months but have had many issues with coverage and high costs.

I would say I am a medium to high complexity patient as I have had many health issues across my body that have required me to make many doctor's visits and undergo rigorous testing. I usually end up seeing at least 2 doctors a month, and due to the nature of my heart issues, I have an unpredictable amount of urgent care visits. Additionally, I know that sometime in the next year I will likely have an appointment with an autonomic dysfunction clinic as well as an appointment with a jaw specialist for painful TMJ.

Ideally, I'm thinking I should get a PPO plan, but open enrollment only shows HMOs available to me. I spoke with someone from United's Copay Select Direct Golden Rule Insurance Company, but I've seen online that it's not considered "real insurance". I would prefer a plan that allows for self-referral and lower copays so I'm not shelling out as much every appointment.

What PPOs or other similar plans are available in GA at a reasonable cost?
