Employer was paying for health insurance but then we got a bill for $11k in unpaid health insurance?

My dad got injured at work almost 2 years ago and was currently on workers comp. About a year before he got injured his employer had agreed to cover my dad’s health insurance which was about $200/week. Now, my dad went back to his job as he was told he can go back as long as it’s modified and fits his medical needs. When he went to his job to sort out scheduling, he was told he has a balance of $6k for his unpaid health insurance and he needed to pay it. My dad asked why he had a balance if the company pays for his insurance to which they couldn’t really give him an answer for. Today, he received a packet in the mail from the company letting him know that his total balance is $11k for unpaid health insurance and he needed to pay it soon. Based on the dates provided on the packet, they started charging my dad once he was placed on medical leave by workers comp. Not once during my dad’s leave did anyone from the company reach out to my dad to let him know he was going to be charged. As mentioned, when my dad went to work for scheduling he questioned why he had a balance of $6k and they couldn’t tell him why he was being charged so I feel like they’re going to give him the run around. Any recommendations on what to do regarding this? Or any ideas as to why he is now being charged? This is in California.
