BSBS billing me for holter monitor test with in-network provider

My Anthem BCBS EOB shows an in-network cardiologist conducted a holter monitor test, total charges $4372, $2234 adjustment, billing me $960. They sent me separate document that says it was medically not necessary, with requirements: "the person must have a heart rhythm event less than once every 48 hours" and "another external test has been done for at least 14 days and has not shown what the problem is." I had three EKG's showing irregular heartbeat, and a stress echo before this holter test was done.

I showed the bill to the provider they said it was wrong and they would appeal. That was 3 months ago and never heard back. I called the insurance company they said they would setup a payment plan for $50/month and send me a new bill for that, but I never got it. I have a letter from a collection agency for the $960 I went to the providers office this week and the receptionist said her records show they wrote me a script for this procedure and they get authorization from the insurance company before writing these. Said her office manager would call me that day to explain. Never heard back

This is NJ and Im 53 years old, salary >$100k. What are my next steps?
