BCBSFL unexpected ‘out of state doctor’ bill for biopsy lab

Hello! I’m not sure what to do about this bill that I think is a mistake.

My in-network neurologist got a pre-authorization for a skin biopsy to detect small fiber neuropathy. They did the biopsy procedure a few months ago and sent it to Corinthian Labs for testing (specialty lab). Now, there was an issue where BCBSFL sent me a form to do for secondary insurance and before I submitted it they started denying my claims until I filled out the form (I do not have secondary insurance). So they initially denied the biopsy procedure (they have since reversed that and paid it this week). While all that was being fixed and while the biopsy procedure claim was denied, a charge for the biopsy lab showed up and was labeled as ‘out of state doctor’ and it’s being treated as an out-of-network charge and BCBSFL are paying barely anything. It’s showing my portion to pay as more than $3500.

From what I had been told/have read from others getting biopsies done through this lab, the charge is somehow supposed to go through my doctor’s office and I shouldn’t be charged out of network like this. I am not sure what to do or who to talk to first to get this fixed. I also noticed that the pre-authorization that was given for the biopsy was later “voided,” which I assume happened when the claim was initially denied (another pre-auth for an iron infusion that was briefly denied too is also “voided,” but has been paid). Maybe that void is causing an issue? I just haven’t had to get something like this fixed ever with health insurance and don’t know where to start.
