Can anyone help me make heads/tails about my Dental Insurance? (DeltaD-CA)

Hi all,

Just for a bit of background:

Insurance: Delta Dental CA – PPO Annual Max: $2500 (In 2023, it was $2000 but retroactively changed in 2024 to $2500, in contract negotiation with employer.)

I am trying to figure out if I was overcharged or if I can get a refund for overpayment.

I had to get 2 crowns last year (one that was normal procelain/ceramic and another that was noble metal infused porcelain). In the end, I had to pay over $1k out of pocket.

There seems to be some sort of discrepancy between the receipt from my dentist and DD. I have a feeling the discrepancy is due to the retroactive change of my annual max from $2000 to $2500.

The links below are images of my statement with personal info redacted/cropped:

I feel like I should be getting a refund.

Any help would be appreciated!
