I need help finding a plan that makes the most sense to me and my situation

I recently moved from ny to sc and I’m looking for new health care services to aid me in my current situation. I have a degrading shoulder and osteoarthritis, I’m 30 years old. I’m seeking care from regenerative Therapy services to hopefully give me a new lease on life but obviously it all depends on my insurance plan. I’ve looked at hmos ppos epos and I have no idea what I’m seeing. This is what I want hopefully someone can steer me in the right direction I want to go into a doctors office and get treated and pay like 10$-150$ without having to pay more out of pocket costs. I’m looking for a cellphone plan basically. I understand that’s not the case and I’ll have to cover some additional costs. I do not mind the monthly premium. Or a high deductible and a low max out of pocket. As long as it’s a one time payment for the whole damn year and still receive services. I just came off of medicaid and it’s the difference is night and day. I went from having almost everything covered to having to find in-network/out-of-network providers. Which is absolutely annoying af. I’m looking at blue cross blue shield right now and don’t know where to start
