How do they know what records to request?
Recently applied for life insurance and received a request for records from a specific provider. I had not mentioned this provider in the application because multiple other medical professionals determined that their evaluation was incorrect and a misdiagnosis, and I stopped seeing that provider after a couple visits. The insurance company knew exactly who the provider was, where to find them, and what records to ask for. I thought they might have gotten this information through the medical information bureau, but based on what I read it seems that the MIB would have to notify me if somebody requested my information. I never received any notification and certainly never authorized anything, and I’m now wondering if there was a breach of my medical information and the insurance company was able to find more detailed information that way.

Edit: it was a term life insurance application. They ended up denying me any coverage, and said it was because of the records from that provider. I tried to provide them with records from all the other providers who corroborated that this was a misdiagnosis. The underwriters said this would not change anything, and it would likely be impossible to get insured anywhere because that misdiagnosis is on my record.
