Can you submit a claim to insurance yourself for gender affirming surgery done at an out of network plastic surgery center?

So I know someone might be tempted to go "haha you CAN but,,,," but I wanted to get some details. I live in Colorado, and supposedly the law here is that insurance has to cover certain gender affirming surgeries – like top surgery. But does that only apply if I'm going to someone who takes insurance? The practice I'm considering doesn't take insurance at all because most of the procedures they do are cosmetic, but I also know that legally, unlike a lot of procedures, it can be considered medically necessary and it's a matter of documentation. Currently I'm just planning on borrowing money for top surgery and dealing with the suffering. But if there IS a chance I could submit a claim for this and get some of it covered (well…what I mean is more get reimbursed for some of it after the fact) even despite it being done at an out of network plastic surgeon, I would want to attempt that. If it's possible to do that, does anyone know how I would go about it and what documentation I would need? If not eh oh well, figured I'd ask.

Also this insurance plan is through my employer (> 100 employees). And neither Google nor my plan documentation is helping me at the moment
