Retroactive “adjustments” to covered visits??

Hello,

My fiancé had been visiting a COVERED program virtually in 2022. It’s a therapy for those with a certain mental illness. It was a reference from her psychiatrist and we called to make sure it was covered, directly to her insurance.

2 years later (and parts of last year) she has seen amounts pulled out of her bank multiple times for “adjustments” to the visits coverage. There’s got to be at least 10-15 times now, without ANY notice that amounts have been pulled. The insurance company sent us off to a call center in “idk what’s going on-ville” and the therapy group did their best on their end to give us the info they had.

At the end of the day I don’t even know where to start, but is it legal for the insurance to retroactively decide not to cover visits? This is probably my first stop… if you have any advice PLEASE drop it here. The treatment was absolutely priceless, but we also were told the price by our insurance and they pulled the “oops my bad not enough” card it seems…
