Same Dr. different location considered out of network.

Hi Everybody, So I recently went to an optometrist at a Sam’s club for an eye exam. The Dr. works at more than one location for Sam’s Club. ( about 20 mins apart) . I was told while I was there that my previous optometrist had already gotten an approval for my next eye exam and until they canceled it, they couldn’t get an approval for the visit. So thinking it would be easier, I just paid for the exam and told them I would submit the paperwork for reimbursement. I ended up calling VSP insurance to ask what forms I need to submit so the person on the line asked for the doctors name and phone number. After a few minutes she says “ I see the Dr. listed as in network for another location but this location is out of network”. Really? Same Dr. same store name but different address makes it out of network? I originally got the info off the insurance companies website so I would go to an in network Dr. can they really have a doctor in network AND out of network at the same store but different locations?
