Coordination of Benefits Unknowingly on Mother’s insurance

Unsure if I have the right flair. This is somewhat of a complex situation so I'll try to explain it as best as I can.

My parents divorced when I was 13 – and I went to live with my dad in a different state (IL). I don't have a great relationship with either parent and never really did. Apparently part of the divorce agreement was that my mother (lives in WI) still cover me under her health insurance. I didn't see a lot of doctors during that time and quite frankly had no knowledge of the insurance situation so I just kind of assumed everything was normal.

5 years later I was going to college in a different state (MN) and was told that I would need a health insurance plan. Without knowledge of having one and with my dad's suggestion I signed up for the college's health insurance (Blue Cross Blue Shield). When college ended, I signed up for health insurance through the state (MN) – which I had for a year and a half before getting a full time job in October 2021 with health insurance (Medica). I stayed at that job through mid August 2022 before starting another full time job at a different company with its own health insurance (BCBS), which i currently have now.

About a month ago I recieved a bill from my doctors office claiming that I owed them $3k from visits in 2022 and that I needed to contact Medica (old insurance) for coordination of benefits. I called Medica and they claimed that I had Blue Cross Blue Shield as my primary insurance during that time in 2022. I explained the situation with starting new jobs and the insurance change (with the start and end dates) and everything seemed cleared up. Then I recieved a voice-mail Friday stating that their records state I had Blue Cross Blue Shield insurance with a start date of June 2020.

I was very confused and brought the situation up to my dad. My dad had no clue, but told me my mom was responsible for all my health insurance so I reached out to her. She told me I was still covered under her health insurance and have been this entire time. Her health insurance was Blue Cross Blue Shield but now has changed.

I never had knowledge of this nor the account information (and I still don't) so I'm unsure how Medica knew this information. Because I didn't know any account information I wouldn't have given my Mother's insurance information to any hospital or clinic.

How do I navigate this situation with Medica claiming that my mother's Blue Cross Blue Shield insurance was my primary insurance for 2022 when I had no prior knowledge of it?
