3 upcoming surgeries. What insurance plan?

I have access to the US Federal health insurance options. This is beyond overwhelming, as we have 3 upcoming surgeries next year. Any help is greatly appreciated.

I've done some rough calculations based on what I can see from OPM's website above, on what I think are the 3 best insurance plans (I could be wrong).

The three surgeries are all covered by insurance. Septoplasty, Shoulder Labrum Repair, and Inspire Implant.

Needless to say the deductible would be hit for the HDHP option.

Here is my math. Please, any input is helpful, even if just to ask questions.

Right now I'm leaning towards the HDHP, but I can't help feel that is the wrong choice. I've always been told that HDHPs are for those who are "happy and healthy", and not those with 3 planned surgeries…
