Turning 26 soon, chronically ill with questions

Hi, I’m turning 26 next month and realized I should probably get my prescriptions in order before I am officially off my parents insurance.

I currently have an active Medi-Cal plan that I haven’t used before. I have only used my parents kaiser plan in the past because it has better coverage and we’ve used them in the past so all our info with them. So I have insurance, but nothings really set up and i’m overwhelmed trying to figure out the next steps.

I have 5 prescriptions that I take regularly and a few specialists I see 1-3 times a year. is there an easier way to transfer my information from kaiser to medi-cal. -Specifically, how do I transfer my prescriptions and manage my medical record? – Do I need to make new appointments with a new medi-cal doctor or can i use my old kaiser prescriptions. -Do i have to start the processes over with new specialists?

Dealing with insurance was already exhausting, if there’s a way to make the shift easier please let me know as I’m not looking forward to this.
