Blood work claim denied; $65 out of pocket is now $900; Blue Cross Blue Shield of Michigan vs. Florida blue; same or different?

To keep it short, I got blood work done at “Quest Diagnostics” and what was supposed to be $65 out of pocket turned to $900 since that blood work apparently got denied by my insurance. This has been a 2 month battle with nothing new from BCBSM.

I did get a new bill statement in the mail today from the clinic “Quest Diagnostics” and I noticed something extremely suspicious to me.

Although they got my Insurance ID and group number correct, it shows the insurance name as “Florida Blue OOS BCB”. The insurance I actually have is “Blue Cross Blue Shield of Michigan”. Although I live in Florida, my insurance is with BCBSM since my employer is based in Lansing, Michigan.

I’m aware that Florida Blue is part of the blue cross blue shield umbrella, but if they put the wrong insurance name, would that be a reason for my claim to be denied?

BCBSM still sent me my EoB and they can see this on their computers from my discussions with customer service, but if it is submitted as Florida Blue, wouldn’t they have a “limited” view of my benefits if they can’t pull up 100% of everything through the Florida Blue name?

I’m questioning if no one else noticed this and this could potentially be the reason my claim was denied. I got this same blood work done not even 3-4 months ago with the same clinic, and that was covered like normal.

Any insight from anyone more knowledgeable on this on whether my suspicions are correct where putting “Florida Blue” instead of “BCBSM”could cause my claim to be denied?
